Prescription Refill Request

*During the week of Jan 27th-Feb 1st, all prescriptions will be filled at our Breton office while our Cascade office is under construction.*

Please complete the following form if you wish to request a refill for your pet's prescription. Please note that a veterinarian from our office must have prescribed the original prescription.

We will respond to your request within one business day by telephone, email or text message if we have any questions. Your prescription will be ready within 24 hours and we will inform you when it is ready for pick up.
